Thank you for referring your patient to Warming Light counselling. Accredited Mental Health Social Workers, like psychologists, use a range of counselling interventions and focussed psychological strategies (FPS) to assist people who are experiencing mental health issues. Clients with a Mental Health Treatment Plan (MHTP) can receive a medicare rebate under the Better Access Scheme when working with me.

Referral Requirements

For a referral under a Mental Health Treatment Plan (MHTP) through Medicare, please include the following details:

  • Date of referral

  • Patient’s full name

  • Date of birth

  • Address

  • Diagnosis

  • Requested services

  • Number of sessions

  • Referring practitioner’s name, provider number, and signature.
